The Giving Neighbors organization performs due diligence on all applications to insure all the
requirements are met. Each item is verified and the need is determined to be valid. We will stay true to the Mission and Vision of the organization.

Monies provided will go to the general fund and this will allow The Giving Neighbors to fund the most urgent need, one with the largest financial requirement or one that can be completely fulfilled. This will be determined by the Team at TGN and the amount of funds available.

N.C. - Texas Collin County - 5.05.08 "I have been in need of a root canal for over a year now. My dentist keeps me on antibiotics in hope of saving a tooth that suppoprts a very important bridge. At one point in my life I had both dental and visual insurance but no longer have either. I have lived with this pain for so long and really would be ever so grateful for your assistance. Even with my temporary job I could not save enough to have the root canal, as my dentist wants to be sure that I have the money before he begins." Note: Letter of reference indicates N.C. has been diligently looking for fulltime employment but has not found anything to date.

A.M. - Texas Dallas County - 4.29.08 Cancer (of the throat) survivor is requesting assistance with dental work. All of the radiation and chemotherapy treatments have left him with his teeth quickly deteriorating. He has health insurance but is still financially devastated from the illness and not being able to work. He has gone back to work one day a week and wants to return to full time work. All of his teeth need to be worked on to stop the continued breakdown. Update 05.07.08 His wife has some insurance coverage. The dentist office is waiting to hear how much that insurance company will pay. We are working to get a final fiqure on this required treatment.

K.&C.S. - Texas Dallas County - 4.14.08 Family of four have incurred many medical bills with two children (3 and under) in the hospital with serious illnesses several times. Both parents work and have health insurance but are still overwhelmed with medical bills which are out-of-pocket expenses. Update 4.29.08 More bills are coming in so we are collecting all of them. Working with the hospital and doctor's offices to negotiate final numbers and payment plans. Itemized bills will be collected and reviewed. Update 05.07.08 Contacted the hospital for itemized bill. Contacted one billing office of one primary doctor and have received a 20% discount on balance due. Other bills still pending.
Request: Assistance with review of medical bills outstanding, work to get medical service providers to work with them for discounts and payment plans so this family can pay out existing bills without suffering financially in other areas of their lives.

K.B.- Texas Collin County - 3.6.08 Little 6 year old girl is being treated for large B-Cell Lymphoma at CMC. Surgery is now required and a deposit is needed of $500.00. The child is not covered by any health insurance or aid at this time.
Status: More medical care and treatments may be required and to date we do not know the amount of the medical care costs. It is estimated costs is in the many thousands of dollars. THANK YOU CHILDREN'S! Although today we have dealt with only the immediate request we are accepting donations to help provide for further medical needs for this child in the future. SPECIAL NOTE SHOULD BE MADE: The staff and students at the elementary school this little girl attends has provided funds through fund raising efforts made by them. Thank you Finch Elementary for being A Giving Neighbor! Update 3.10.08 - The 500.00 was donated for the deposit for the surgery on Fri. 3.7.08. The Actual Amount now listed is the charges owed up to 3.7.08. Update 3.19.08 K.B. surgery was performed and according to a family member was home on Saturday and doing well. K.B. is scheduled for chemotherapy treatments beginning tomorrow at CMC. She will be in the hospital for 5 days receiving treatment. A hearing checkup is scheduled towards the end of the month to discover how her hearing has been affected. Thank you to all who have donated so far! Update 4.14.08 K.B. has recovered from her surgery and is scheduled to be back at school in May. Follow ups at CMC have been done and currently K.B. is doing well. J.W. - Texas Dallas County - 3.5.08 She is asking for help to get a top molar fixed. She has lost a filling and either needs a crown or a root canal and crown. She currently has no way to pay for this medical need to be met. This situation became urgent as pain set in.
Status: Update - 3.19.08 she has had the root canal done and is pending the crown. Other dental work required will be determined at a screening next week. 3.28.08 root canal and tooth build up is completed. Crown is scheduled for June.

A.D. - Kentucky - 1.16.08 - - She writes "I am very desperate need of having all my teeth pulled. I just came from the ER about an hour ago and the doctor said if I continue to let them go the way they are that it would seriously hurt my health. My jaws and gums stay swollen all the time. I have tried everywhere for help. I have no income and no insurance. I can't get a job anywhere cause my teeth are broken and black. I would love it so much if anyone could help me out. I can't get a medical card or anything. I desperately need help. I want so bad just to be able to smile again. I have severe anxiety and panic attacks and I am just so afraid of the dentist and I'm afraid I'm going to die if I don't get help. A person I knew died and they said it was from bad infection in the mouth went to the brain. I don't know what to do anymore. Please help me."
Status: We are searching for a dentist to accept this Neighbor in Need and working to find out costs. Emergency room documents support this need. She has been prescribed an antibiotic but did not have the funds to get it filled. We will work to help her get that filled.Update 1.21.08 Finally found a pharmacy in this small town to work with us to allow TGN to pay for the prescriptions she needed. The antibiotic and pain medications she has taken. Update 1.30.08 We have found an oral surgeon who has agreed to see A.D. Thank you Lord! Her initial appointment is scheduled for 2.11.08 and cost is $200.00 which includes xrays. At that time we will know the requirements from the doctor and find out what the further actual costs will be. Update 2.13.08 Oral Surgeon saw A.D. yesterday and determine all of her remaining teeth (25) must be removed. The total cost reflected below is said to be a discounted price and must be paid in full prior to surgery. This will not include any denture work. Update 2.20.08 She is getting a second opinion and has an appointment on 2.28.08. X-rays have already been taken so these will not have to be retaken. Update 3.5.08 Second opinion offers us the surgery for $3442.00 if paid in full. Update 3.19.08 A.D. lives in a rural area of KY and is not physically close to a large city so our options have been limited. She is now scheduled for surgery on 4.8.08. Her teeth will be removed in four different surgeries, two weeks apart. She will be out of pain and infection once this is completed. She is very anxious about this process and asks for your prayers. Update 4.14.08 She had her first surgery on 4.8.08 and all went well. Next surgery is scheduled for 4.23.08. Update 4.29.08 Final payment has been provided for the last two surgeries. So far all has gone well and recovery has been good.

M.H. - Texas Dallas County - 8.13.07 - bedridden from MS she is requesting a handicapped equipped minivan for transport to and from doctor's appointments. She lives with her daughter, son-in-law and family and is cared for by them. They need a way to provide this transportation on a regular basis. They are not in a financial position to be able to purchase this item. As you can imagine the requirements of this family is huge. Since she is bedridden and has been for many years a regular car is out of the question. She must be transported in a wheelchair and they need a lift to be able to get all required into the van.

C.F.- Texas Dallas County 10.25.07 - Had to quit her job due to floor-of-mouth cancer. A tumor was removed and all of her teeth were removed. Upon recovery she needs to look for another job and needs to have dentures to do that.
Status: Needs a full set of dentures. Must find a dentist to assist with this situation. 11.8.07 A dentist has seen her and said some further healing must take place but she is a candidate for dentures and will provide the care for this need. Next appt is 12.10.07 Update: 12.28.07 Impressions were taken at the last visit and the final product should be presented mid to late Jan. Update 1.18.08 additional fittings have been completed and adjustments being made. Still on schedule for late Jan to receive the final product. Update 1.30.08 she received her dentures last week but found they were not a fit. She saw the dentist again on 1.28.08 and the doctor is sending them back to the lab to be redone. Thank you Dr. for making sure these are absolutely right! Update 3.19.08 she has received her final set of dentures and is thrilled. She has been able to go to work.

J.R. - Texas Collin County 2.11.08 Contacted by school nurse and parents for assistance for 9 year old J.R.. He needs an ADHD evaluation with his doctor and 30 days worth of medication. His parents are currently unemployed and need cannot fund this need. The school nurse has indicated the child is struggling in school and needs this help. Update 2.20.08 The child went was seen at the doctor's office today for a flu or virus. They provided him with help with that as well as providing the ADHD medication.

J.B. - Ohio - 7/23/07"I lost almost everything I owned in the flood of 2004. Due to me being in a flood zone my belongings was not covered by insurance. I also contacted Black Lung disease from being around so much mold. Although I took care of my teeth for several years when I had full insurance, I now have no dental insurance. Due to my illness, my teeth has become very weak, from my illness, meidcations, and age. Due to several financial reasons, I need to return to work. I would like to return to teaching special needs children on a part-time basis because of my health. Unfortunately, I have a front tooth missing and two on the side. Besides having difficulty eating, my smile hinders my self esteem. I have worked all my life serving people and helping them physically, emotionally, ecomically and with their education. Now, I have to humble myself and ask for assistance to imporve my health." 8.13.07 UPDATE - Saint Elizabeth's Health Center (Dental Clinic) will provide an implant to the front of her mouth for the requested amount of $900.00. She also needs to have Posterior rehabilitation with upper and lower removable partial dentures. The cost is $704.20 each.
She is requesting the funds for a tooth implant in the front. 8.24.07 Update The implant is scheduled for Sept. 11th.

M.K. - Texas Dallas County - 1/30/07 Request for dental services has been made. She went to a dentist for evaluation and her quote for all the work to be done was $13,132.00 (retail prices). We are working to get assistance for the critical dental care needed. Three root canals/crowns and two extractions are critical. UPDATE 3.8.07 - M.K. went for her first visit, exam, xrays and treatment plan. Next schedule visit is June. Financials will be provided as we go but the current actual need is below. Thanks to Dental Health for Arlington! 4.2.07 Update We are working with Baylor University Dental School to assist with some of the dental work to be done. 5.6.07 Update - Next appointment is June 14th. TGN will provide funding for that appointment. 6.12.07 UPDATE - Confirmed appointment for June 14th with Dental Health of Arlington. Funding of $140.00 approved for two fillings and office visit. 6.25.07 update working with dentist office to get all work completed and the final total costs with a 15% discount provided. 6.28.07 update Since the work required is so extensive we will work with as many options as we can, that leads the actual need to a wide range. Work has begun and we are excited about getting this young mother's dental health completed. 7/17/07 UPDATE a dentist in the area has agreed to provide a 15% discount. This dental work will be shared between DHA and the dentist in Rowlett, TX. 8.13.07 UPDATE - M.K. has been treated for decay at DHA (Dental Health for Arlington). They indicated her wisdom teeth must be removed and an oral surgeon at the Baylor Dental College was recommended. She had her initial appointment with the surgeon and surgery is scheduled for 8.23.07. The cost is $425.00 8.24.07 Update Surgery was completed and she is now in recovery. M.K. -Texas Dallas County 1/30/07 Single 23 year old mother of a 7 month old son went to a local emergency clinic in great pain. She was told she had to have a CT Scan. A call was made to The Giving Neighbors for assistance. Working with the doctor at the clinic we found assistance to receive this scan at cost for $350.00. Although, that is FANTASTIC she continued in pain and had to wait 24 to 48 hours. Medications were provided. She has been treated but diagnosis is still unclear. They were looking for kidney stones and appendicitis. Appendicitis was ruled out. Stones might have passed during the waiting while she was on pain medications. She works part time trying to be with her son as much as she can.

V.B. - Colorado 5/7/07 She works waiting tables and does not currently have medical insurance. She was in severe pain so she went to the emergency room at Denver Health. She was diagnosed and treated for a kidney infection. She is requesting help with her bill of $526.00 of which she has paid $105.00. We will contact the medical service provider to determine if a discount or payment plan is possible.
Status: 5.8.07 We contacted the service provider and they recommended she apply for assistance from the state for the reduction of this bill. She agreed to apply. Once application is approved we will review the financial need again. 5.24.07 She applied for assistance and we are waiting status. 6.5.07 UPDATE V.B. is in the process for assistance from CHIP. 6.12.07 Final status is still pending. 6.17.07 UPDATE No funding was required. She was accepted to the program for financial assistance via CHIP in Denver so the bill was forgiven thru the program.

P.G. - Texas Collin County 5/25/07 "I am requesting assistance due to lack of health insurance and a need for doctor visits and prescriptions. I was recently laid off. I have a Psychiatrist visits that cost $60.00 per visit and I have prescriptions that are too expensive for me to fill. One of my prescriptions cost $56.00 and I am waiting to find out if my doctor can change my meds as of now the other prescription is $450.00 per month to have filled. Thank you so much for the services you provide."
Status: This young women is working with the doctor to get this new prescription change to something more affordable. Another doctor's appointment is scheduled for Wednesday. The Giving Neighbors will fund that appointment. She provided payment for the prescription of $56.00 this month. She will contact TGN on Wednesday with an update. 6.5.07 UPDATE The doctor's office provided 1st months medication for free $450.00 value and is helping her to make application to Pfizer for permanent assistance on this required medication. Another doctor's appointment is required next week on June 12th. TGN will provide funds, $60.00, for that appointment. 6.11.07 UPDATE The Mech Center provided assistance to P.G. for medication. The financial assistance requested of $450.00 is not required at this time. Request for assistance for this weeks doctor's appointment of $60.00 was funded.

J.B. - Texas Collin County 6.11.07 This young man was rushed to the emergency room with a severe migrane headache. He received treatement at a nearby emergency room on 5.24.07. He again was rushed to the emergency room on 5.29.07 still in severe pain and vomiting. Tests were run (spinal tap) and nothing was found. They recommended he follow up with a Neurologist to determine what was going on. He,nor his family, had the funding to see the Neurologist. They have come to TGN for help. 6.11.07 UPDATE TGN made contact with the Neurologist recommended, they have provided a discount and an appointment has been made for J.B. to be seen today. 6.12.07 update - Migraine headaches were diagnosed, medication was provided and all answers to questions provided.

B.C. - Texas Collin County 5/07/07 "I recently underwent surgery for the removal of a malignant GI stromal tumor. Follow-up care involves seeing an oncologist regularly, having CT scans done quarterly, and going on a drug called Gleevec. We are currently uninsured." Both he and his spouse are employed but the costs for the ongoing treatment are astronomical. Approx. costs for monthly Gleevec depending on dosage is $780.00 to $2400.00. Oncologist has not yet been seen.
Status: WE need funding to assist for his monitoring and recovery. We are seeking medical service providers who will provide assistance with funding on these necessary steps for his recovery. 6.12.07 UPDATE No further requests or status has been provided to TGN. We consider this request completed.

T.S. - Texas 5/08/07 Recovering from cancer treatments he is requesting assistance of a month's worth of prescription medications. Lost insurance. They are appling for prescription assistance from the pharmaceutical companies.
Status: Working with the pharmacists. She is contacting the doctor asking if other (less expensive) medications might be substituted. Pending call back from doctor. One of the doctor's finally contacted the pharmacy and approved other medications. This cut the cost for T.S. dramatically and will allow them to get these med's much easier. We are still waiting to hear back on one more medication. 5.24.07 Finalized last prescription requested. The pharmacy made recommendation of a comparable prescription which the doctor authorized.

J.D. - Texas 5/16/07 He was treated in the ER on 5/7/07 and released with three prescriptions to be filled. He is working but needs medications to be able to get well and fully recover. An antibiotic, antihistimine and inhaler have been prescribed.

D.R. - Texas 4/2/07 She has requested a doctor's appointment to review her blood pressure and to provide medication. In the past she was diagnosed with high blood pressure and has received care thru community clinics. She is also requesting a physical and mammogram. Currently, she has no refills on prescription medications and is suffering from constant headaches. 4.17.07 Update - InstaCare Clinic in McKinney provided full blood work up and office visit for diagnosis and treatment. They work to provide low cost health care. The high blood pressure has been treated and medication has been prescribed. Follow ups will continue here for D.R. She said she was feeling better and very blessed.

C.W. - Texas 11/15/06 Infection in mouth and in pain. Needs 3 root canals with crowns, 1 extraction, a partial plate in lower part of mouth. She is on a blood thinner from a recent heart attack and needs special monitoring during any of the procedures above. We are look for that special dentist. 11/27/06 Infection proceeded to increase. That SPECIAL dentist has provided assistance (Thank you) and the treatment required is, antibiotics, 9 extractions, 1 root canal, full denture, 1 partial denture.
12/14/06 C.W. is feeling better, infection is under control. Extractions and Root Canal still to be done. Waiting for infection to clear. 12/20/06 Infection cleared, worked planned to proceed. UPDATE 1.12.07 - root canal is done. appointments made and planned to be finished by 1.31.07 Help give the gift of a healthy mouth and cosmetic changes to ensure a job can be obtained.1.25.07 Impressions will be made on 1.26.07. Next are extractions and receipt of dentures 2.13.07. Thanks to ALL that have donated! 2.19.07 Unexpected issues with the lab has hindered progress. Appointments moved and final results still pending. Next appointments 2.19.07 and 2.28.07. UPDATE 3.7.08 - All dentures are back from the lab. New appointment for extractions planned for 3.28.07. During these months she has fought, lab not returning products when scheduled, infection and pain. She was able to get antibiotic shots which provided great relief. We are praying for an earlier date for extractions and putting in the dentures. 3.15.07 Dentures received. Final surgery scheduled for 3.28.07. 4.2.07 Update Surgery was completed and recovery is moving forward. All current bills fully funded. Dentures must be adjusted.

K.P. - Texas Hearing Aid- K.P. was in a car accident, hit by another car on the right side. The person driving the car that hit him was not insured. He suffered minor injuries but was alright. He did have his hearing aid in his right ear knocked out of his ear and it was never recovered. He uses a hearing aid in both ears and is extremely hard of hearing. I.N. - Texas I am a single female, 48 year old hairdresser, who has chronic muscle and joint pain for the last several years. I have sought out doctor after doctor to no avail in getting any help. Then I have had chronic ear problem of constant fluid and sinus infections. I have noticed that if you don't have health insurance doctors have a tendency not to do much for you. I have lost 30% of my income over the past few years so I am living paycheck to paycheck. I want to be able to go to a doctor for a diagnosis and resolution to these problems. G.G. - Texas 12/21/06 I was employed for 4 years and because of my health I was fired. I was in the hospital 2 weeks ago and I have several health conditions that keep me from going back to work right now. I do plan on getting well and working. My daughter's name is G. and she is a very gifted athlete. Even though she's a freshman this year, she will be starting the softball season as a varsity player.
